major advantage of the Provider Alliance is the streamlined
application process. Once program eligibility has been established,
accounts will receive a formal proposal within 72 hours
of our underwriters receiving a complete application submission.
To initiate the application process (only for eligible risks),
follow the instructions below:
| Application Instructions |
- Print and complete the Provider Alliance Application
for Insurance. All requested information must be provided
and the application must be signed in order for your submission
to be processed.
- Gather the supporting documentation required for the insurance
submission. A complete
list of required documentation can be found on page
7 of the application.
- Send or fax your COMPLETED SUBMISSION to the designated
Provider Alliance agent.
- Once our underwriters have received your complete submission,
you will receive a proposal for insurance within 72
hours.
- Binding requirements, payment terms, etc., will be presented
by your agent upon your acceptance of the proposal.
