One major advantage of the Provider Alliance is the streamlined application process. Once program eligibility has been established, accounts will receive a formal proposal within 72 hours of our underwriters receiving a complete application submission.

To initiate the application process (only for eligible risks), follow the instructions below:

Application Instructions
  1. Print and complete the Provider Alliance Application for Insurance. All requested information must be provided and the application must be signed in order for your submission to be processed.
  2. Gather the supporting documentation required for the insurance submission. A complete list of required documentation can be found on page 7 of the application.
  3. Send or fax your COMPLETED SUBMISSION to the designated Provider Alliance agent.
  4. Once our underwriters have received your complete submission, you will receive a proposal for insurance within 72 hours.
  5. Binding requirements, payment terms, etc., will be presented by your agent upon your acceptance of the proposal.
